Pediatric dentistry and orthodontics are different specialties, and most children who need orthodontic care eventually see two clinicians: a pediatric dentist for routine care, and an orthodontist for braces, aligners, or early intervention. At Nickel Pediatric Dentistry, when your child is ready for that orthodontic conversation, we refer them to Dr. Zoey Orthodontics - a board-certified orthodontic practice located next door to our office, run by Dr. Zoey Gutierrez-Nickel, a Vero Beach native and Dr. Andrew Nickel’s wife.
That’s not a casual referral. It’s a coordinated handoff between two specialists who share patient context, a clinical philosophy, and the same commitment to taking care of Indian River County’s families.
When Does a Child See an Orthodontist?
The American Association of Orthodontists (AAO) recommends a first orthodontic evaluation by age 7. That doesn’t mean your child will start braces at age 7 - most kids who eventually have orthodontic treatment do so during adolescence - but the age-7 evaluation gives the orthodontist a chance to identify any developing issue early, when intervention (if it’s needed) is much simpler.
At Nickel Pediatric Dentistry, Dr. Nickel monitors orthodontic development at every routine visit. We watch for:
- Crowding as adult teeth come in
- Crossbite, overbite, or underbite developing
- Habits affecting bite (thumb sucking, prolonged pacifier use, mouth breathing)
- Jaw growth patterns that suggest early intervention may help
- Airway-related orthodontic concerns (palate width, tongue posture)
When we see something that warrants an orthodontist’s evaluation, we make the introduction.
How the Referral Actually Works
Most “referrals” between dental practices look like this: a phone call to the receptionist, a handed-off paper packet, a starting-from-scratch consultation. Ours is different.
When we refer your child to Dr. Zoey Orthodontics:
- Dr. Nickel speaks directly with Dr. Zoey about what we’re seeing and why we think an orthodontic evaluation is warranted now.
- Your child’s dental records, X-rays, and growth history transfer seamlessly between offices - nothing has to be re-explained, re-imaged, or re-collected.
- The orthodontic consultation starts with full context. Dr. Zoey already knows your child’s dental history, eruption pattern, and any habits or concerns we’ve been tracking.
- Future coordination continues. Even during orthodontic treatment, your child still sees us for routine cleanings and any non-orthodontic dental needs. Dr. Zoey and Dr. Nickel coordinate on anything that overlaps.
That continuity isn’t something you can replicate with arms-length referrals. It’s the closest thing to a multi-specialty practice you can get without the corporate-dental-office feel.

Why Dr. Zoey?
Dr. Zoey Gutierrez-Nickel is a board-certified orthodontist who completed her orthodontic residency at the University of Southern California. She grew up in Vero Beach, returned home to practice, and offers comprehensive orthodontic care for children, adolescents, and adults - braces, clear aligners (including Invisalign), early Phase 1 treatment, and the Orthodontic Supervisory Program for children not yet ready for treatment but who need monitoring.

What About Phase 1 (Early) Orthodontic Treatment?
Phase 1 orthodontic treatment is care provided to children who still have some of their baby teeth - typically between ages 7 and 10. It addresses specific developmental concerns (crossbite, severe crowding, jaw growth) before all the permanent teeth have come in. Not every child needs Phase 1 - most kids who eventually have braces do so as a single comprehensive treatment in adolescence.
When Dr. Nickel notices something that suggests Phase 1 evaluation might help, we’ll mention it and refer to Dr. Zoey for the orthodontic perspective. The decision about whether early treatment is the right path is hers - and yours.
